FORM INSTRUCTIONS:
The following forms may be completed online.  However, the state of Alabama Department of Human Resources requires original signatures on all registration forms. Please print out your child's forms and include an original signature on the copies that you supply to the center.

2) PRINT a copy to bring to the center with you.
