FORM INSTRUCTIONS:

The state of Alabama Department of Human Resources requires original signatures on all registration forms. Please print out your child's forms and include an original signature on the copies that you supply to the center.
 
In addition to the following forms, we require a current State of Alabama immunization card for every child.  
 
1) Complete the two page pre-admission form and print it. Be sure to complete all "signature" areas
2) Print additional forms as applicable to your child's situation 
3) Bring the original copies with you to the center
 
Your completed registration packet should include the following items:
 
*    child's pre-admission form (2 pages) with original signatures
*    medical information (to be completed by physician)
*    Current State of Alabama immunization card
*    Developmental history forms (either infant/toddler or preschool/schoolage)
*    Authorization to photograph
*    Regions department form
*    Handbook signature page
