Enrollment Form Completion Guidelines

Thank you for your interest in enrolling your child in one of TodayCare Children’s Centers.  To complete the enrollment process for each child, you will need to print, fill out, sign and return a number of forms to the Center Director.  The first four must be completed for each child enrolled while the others address specific ages or situations. Please complete all applicable forms and return them prior to making a reservation for your child.  All children must have completed enrollment forms including an immunization record before they can be left at the center.

2. Immunization  Record (this is obtained from your pediatrician)
3. Medical information
4. Photo Authorization
5. Developmental History for Infants and Toddlers - If you have a child less than 36 months of age
6. Developmental History for Older Children - If you have a child 36 months through school age
7. Special Care Plan - If the child has special needs
8. Tennessee Department of Health Pre-School Immunization Certificate (Sample - Provided by Doctor)
9. Tennessee Department of Health School Immunization Certificate (Sample - Provided by Doctor)
